Yes, Rhode Island Colony would be correct.

The big question is when do you start using U.S.A.? July 4, 1776 when the Declaration of Indep. was approved, or 1777 when the Articles of Confederation was approved. Based on the following article in Wikipedia, I would not put U.S.A. as the birth place unless the person was born on or after March 1, 1781.

"The thirteen states were formally thirteen independent countries ("states") until ratification of the Articles, proposed in 1777, was completed in [March 1] 1781; at that point the "United States of America" legally came into existence as a union with a confederation form of government."
