How long to get SS App record?

It's been about 10 weeks since I sent in a letter (printed off of Rootsweb) and check for a SS app record. Check was cashed, yet nothing has come. Now I am trying to second-guess what I may have done wrong. I've never had a problem before. Was I supposed to include a self addressed stamped envelope? Any suggestions what I need to do to follow up on this? Thanks.
