      <title>Deceased NOT listed in SSDI - why?</title>

      <description>My father died in 2006.  He is not listed in the SSDI, and believe me, he had a SSN.  My mother says the funeral home assured her that they did, in fact, report his death.  However, he's not in the SSDI. My mother is not interested in pursuing the matter.  &lt;br&gt;&lt;br&gt;Why isn't he in there?  &lt;br&gt;&lt;br&gt;I have searched the index for over a year now, with every conceivable spelling/misspelling of his name, his SSN, dates for b/d, every way I can think of.  &lt;br&gt;&lt;br&gt;Does it matter that he's not in the SSDI?  Could this be a sign of some identity theft or something?  I want to make sure that my mother is not vulnerable to any fraud or ID theft activity.  </description>

      <title>How long to get SS App record?</title>

      <description>It's been about 10 weeks since I sent in a letter (printed off of Rootsweb) and check for a SS app record.  Check was cashed, yet nothing has come.  Now I am trying to second-guess what I may have done wrong.  I've never had a problem before.  Was I supposed to include a self addressed stamped envelope?  Any suggestions what I need to do to follow up on this?  Thanks.</description>

      <title>SSN not listed on Death Certificate</title>

      <description>I have a copy of a relative's death certificate. She died in 1963 (Michigan), but a SSN isn't listed. Is that possible? Btw, she was born in 1867 in Michigan. I would have thought by 1963, that most if not everyone, would have had a SSN.</description>

      <title>Would there be an application if aunt used hubby's SS?</title>

      <description>Would my gg-aunt have needed to fill out her own separate application for SS if she was using her husband's SS?  On her death cert, it listed her hubby's number with a dash and an A on the end.  Does anyone know what this means?  She was in a nursing home when she died, and I believe her hubby's SS was paying for the stay.  If she had to fill out her own application I wouldn't mind sending off for a copy of it, but I would hate to spend $27 and receive a copy of her hubby's!  </description>

      <title>Assumed name question</title>

      <description>My dad changed his name when he was about 20 (from James Roach to James Perry), not legally but by just assuming a new one, and I have his SS-5 with the assumed name. He listed his step-father on it instead of his real father. He was 20 years old and I doubt he went that long without a job. (Did they HAVE to have a SS number to work between 1937-1947 or was it voluntary?) &lt;br&gt;&lt;br&gt;I would like to know if he had a previous SS number under his real name but I don't have a number and can't produce documentation of death because his death cert is under the assumed name...same reason I can't get a copy of his real birth cert (he also filed a delayed birth under his assumed name many years later in a different state than where he was actually born, after establishing his new identity with his new name, including Army records).&lt;br&gt;&lt;br&gt;I have circumstantial evidence of his name change (various certificates, obituaries, cemetery records, newspaper legal notices) but I don't know if the SS Administration would accept that along with his death certificate under the assumed name to prove that it's him and he's dead.&lt;br&gt;&lt;br&gt;I've been unable to find him or his mother in the 1930 census. She left him with his grandparents for a while but he's not with them in 1930. &lt;br&gt;&lt;br&gt;Any advice or ideas?&lt;br&gt;&lt;br&gt;Thanks.&lt;br&gt;Jo</description>

      <title>Question about my dad?</title>

      <description>My father was born in Quincy Ma Feb 17, 1908. His parents were Alfred Nelson and Sylvia M Piispanen who had both been born in Finland, but lived in Quincy Ma.  Dad had gone to Northeastern Univ in Boston Ma.  I tried to find his social security information, and learned his card had been issed in New York.  That is a real mystery to me and my family since he never left MA, that I knew of.  He was in the co-op program at Northeastern, was a civil engineer.  I am wondering if anyone can help me untangle this mystery.  Soc Sec cards didn't exist until around 1936(I think)so he had to be somewhere in NY then.  Maybe someone can help&lt;br&gt;He married my mother Margaret Bradley in Salem Ma on April 23, 1939, and never left Ma again except for a 6-8 month period when we all lived in VA.  &lt;br&gt;I am sorry if this is confusing, but its a confusing situation for us to(my sister and I)&lt;br&gt;Any help or suggestions would be greatly appreciated.&lt;br&gt;Mom never mentioned his being in NY.       <description>I could really use your advice/suggestions:  I have found my Great Aunt on SSDI.  However, SSDI does not list where she died only the last residence.  I have researched the area (Cobb Co., GA) and she did not die there and is not buried there.  &lt;br&gt;SSDI:&lt;br&gt;Bitha Weber&lt;br&gt;346-12-0230&lt;br&gt;Last Residence: Smyrna, Cobb, Georgia&lt;br&gt;b. 7-31-1903&lt;br&gt;d. Mar 1974&lt;br&gt;NOTE:  I have her birthdate as 7-31-1902&lt;br&gt;  I have recently found her youngest son.  However, he is elderly and struggling to remember things.  He does not have an obit.  He said she died in St. Francis Hospital in Peoria, IL and is buried somewhere in Peoria, IL. He can't remember the name of the cemetery.  I asked him if there was a landmark near it and he thinks its near Madison Golf Course. However, I cannot find a death listing for her or a cemetery listing for her.....but my resources are limited.  I did write to the Peoria, IL Library but they weren't able to find an obit.&lt;br&gt;I do know that her oldest son was living in Smyrna, Georgia so its possible she was out there visiting and had her benefits checks sent there and then went back to Peoria, IL and that is where she died. &lt;br&gt;I know that the SSDI can supply me a copy of her Soc Security application.  However, this will not be of help to me as it won't tell me when she died or where she died.&lt;br&gt;My question is....wouldn't the social security office have a date of death and place of death to close out her benefits?  Will they supply that information to a genealogical researcher?  What document should I request from them? &lt;br&gt;&lt;br&gt;NOTE:  Its possible that the date SSDI has for her passing is incorrect as I've found typos for other family members who I know personally their dates of birth/death. &lt;br&gt;&lt;br&gt;Any advice will be greatly appreciated!&lt;br&gt;&lt;br&gt;T.       <title>Obtaining "Cause" of Death...</title>

      <description>I'm hoping someone can point me in the right direction here.  I am trying to find out the cause of my fathers death.  I was raised (with much love mind you) by my mother and never had any contact with him (I've not even seen a picture).  I recently found out he died in 1999 through the social security death index.  I know where, but would like to know how (for my own medical ease of mind.) I understand that Death Certificates do not always (do they ever?) reveal the cause.       <title>FOIA Access to info beyond SS-5</title>

      <description>Does anyone have experience requesting FOIA information from Social Security beyond a copy of the SS-5 application?  The SSDI lists a birthdate for my mother that is substantially different (month, day, not only year) from the date she always gave, which was the date she used on her SS-5.  I would not pursue this except that the date on the SSDI is more likely correct than the date she used, based on the available censuses.  Since it has been impossible to locate her birth certificate at the appropriate archives, I am assuming the SSA has some documentation leading them to disregard the date my mother entered on her SS-5, and I would like to get a copy of such documentation they may have in her file.</description>

      <title>Place of death?</title>

      <description>Hi,&lt;br&gt;I'm new to geneology and I don't quite understand the SSDI.  Someone told me that SS administration requires a certified death certificate in order for someone to be listed in the SSDI.  I found my half brother in the SSDI and it listed his place of death as Massachusetts.  I found an obituary for him that said he died in North Carolina.  I tried to purchase the obit, but the Charlotte Observer website seems to have problems so I couldn't.  How can I find out where he really died?&lt;br&gt;&lt;br&gt;Thanks!&lt;br&gt;&lt;br&gt;(My half brother is Joseph E. Dunn, by the way.)&lt;br&gt;&lt;br&gt;Stfunny</description>

      <title>SS5 and Random Names</title>

      <description>I actually have two examples of this -&lt;br&gt;&lt;br&gt;My grandmother's maiden name was Veto.  I had so much difficulty finding her family in census records until I searched by first name.  I found them - right where they should be, with a last name of Guidone.  Odd, I thought.  Go back ten more years - still Guidone.  Hmm...maybe Veto isn't right.  I found my great-grandfather in SSDI listed as Francesco Guidone.  Aha! I thought.  Maybe if I get his information from SS I can see where the confusion occurs.  Naturally, instead of clearing anything up, I am more confused, as the name on the application is VETO.&lt;br&gt;&lt;br&gt;Second, I requested a printout and application for a cousin.  The printout of information clearly lists him as Luigi Raimondi (also thus in SSDI) but his application clearly states Louis Raymond.&lt;br&gt;&lt;br&gt;Where might Social Security be getting these names if not from the application, and do I have the ability to access these items?&lt;br&gt;&lt;br&gt;Thanks!</description>

      <description>FYI - I recently paid my $27 to the SSA for a copy of my ancestors SS-5.  I was expecting to receive a copy of the original application as I have in the past, but instead was given the NUMIDENT version (which normally would cost $16 if this was my choice).&lt;br&gt;&lt;br&gt;A form letter was included that stated that the SSA has informed and cleared this with major agencies who often request Social Security information for death, birth, passport identification, etc. that the SSA will now ONLY be providing the computer generated style NUMIDENT and that this doucument would be offically sufficient for all needs.&lt;br&gt;&lt;br&gt;I was happy to have my ancestors parents identified/confirmed by this document, but was dissapointed that I did not have a copy of my ancestors own handwriting/signiture.&lt;br&gt;This feature is important to us genealogists!&lt;br&gt;&lt;br&gt;Just thought I'd let the community know, don't expect to get copies of the original anymore :&amp;lt;&lt;br&gt;&lt;br&gt;-Janet&lt;br&gt;</description>

      <title>1 Person; 2 SSA Numbers; 2 Death Locations</title>

      <description>Ok, I will start off by saying this WILL be confusing...&lt;br&gt;&lt;br&gt;My grandfather, Thomas Other Clark also went by Other Thomas Clark. He was born 6-July-1915 (Certified birth certificate from Oklahoma). According to other documents of his, he was "born" either in Texas or Oklahoma.&lt;br&gt;&lt;br&gt;He also has two different social security numbers tied to him. One actually shows the last residence in IL (where he died). But the other is different; "VA", which I am assuming is Virginia.&lt;br&gt;&lt;br&gt;Both SSA have the same birth date, same parents, and the SSDI has the same death date.&lt;br&gt;&lt;br&gt;Any ideas? I attached the SSAN documents as a visual. I have his SS Application somewhere where he states his birth as Jackson County, Illinois.&lt;br&gt;&lt;br&gt;Would he have had to PROVE his place of birth for a special or delayed birth certificate as did my GGmother who presented two personal affidavits and one official census record showing her living with her parents in her birth city county and state.&lt;br&gt;&lt;br&gt;What I'm getting at is .... I'd like to order his birth certificate if there is one as it would reflect his documents of proof of birthplace.  I'm assuming he would have done this in the state of Illinois.       <description>I have obtained the NUMIDENT information for a previously unknown relative and the data contains some internal codes that if I understood them would be helpful. Can anyone tell me what the following means?&lt;br&gt;&lt;br&gt;ETC: L&lt;br&gt;&lt;br&gt;CLD: 03/25/1966&lt;br&gt;&lt;br&gt;DOA: Torrance CA &lt;br&gt;&lt;br&gt;CYD: 03/16/1966&lt;br&gt;&lt;br&gt;ETC: T&lt;br&gt;&lt;br&gt;EDR: N&lt;br&gt;&lt;br&gt;CYD: 06/02/1993&lt;br&gt;&lt;br&gt;I am hoping that some of this may help pin down where he died as he does not appear in CA state or county death records. He may have died out of state and one of these codes may contain that information. The rest of the data provided was self-explanatory and so I have not included it in this post.</description>

      <description>Good morning,&lt;br&gt;&lt;br&gt;I haven't researched within the USA for a few years, but was quite surprised to find that ancestry is now charging to see the index. &lt;br&gt;&lt;br&gt;I sent a Christmas card to an elderly cousin of mine in California and it was returned. Am having trouble contacting anyone who knows when she passed away as this was a cousin I had made contact with through my genealogy research. Is there anyone checking in to this board who could tell me the information from the index? I don't want to order her papers, just the information listed in the index. &lt;br&gt;&lt;br&gt;Her name was Agnes Wolfenden and she passed away in California.       <description>Does anyone know if the Social Security/ birth certificate, etc., keep track of who asks for information on an individual?  I imagine that Bernice June Foster, born 9-27-26, had to ask for a copy of her birth certificate at some point in her life.  Even if she was adopted by another family and had a name change, would she be able to get a copy of her original birth certificate and would the record department keep track of the address of where they sent it to?       <title>How does ones name get into SSDI?</title>

      <description>Here is my question.  How does ones name get reported to SS when they die?  My delima:  My grandfather William Hayes, DOB April 1900, Born and died in Michigan, worked well into his 70's.  I am guesing that he must have had a Social Security Number but he does NOT show up on SS Death index.&lt;br&gt;&lt;br&gt;Could SS not have been notified of his death?  I don't quite understand how the system works.&lt;br&gt;&lt;br&gt;Can anyone shed some light on this??</description>

      <description>My grandmother, Nora Myrtle Sparks Brown, died Nov. 22, 1969, in Oklahoma.I have not been able to find any record of her Social Security number, nor have I been able to find her on the death index.&lt;br&gt;&lt;br&gt;She received a pension from my grandfather, who had been a railroad worker, from the time he died in 1942 until her death.&lt;br&gt;&lt;br&gt;Is it possible that she never HAD a Social Security number?&lt;br&gt;&lt;br&gt;Her number is not on her death certificate, and neither the nursing home where she lived nor the funeral home has records that far back, as both have changed hands since then.&lt;br&gt;&lt;br&gt;Any insight would be appreciated.&lt;br&gt;&lt;br&gt;Jayne</description>

      <description>Amen!&lt;br&gt;They are apparently not paying to get the updates because they are expensive. They used to get them for free if they provided free access to users of their website. That came to an end in April 2004. The Gov't charges everyone now to get updates, including these guys! So they're operating on the cheap!&lt;br&gt;Good Luck.&lt;br&gt;</description>

      <description>I agree that the internal code probably is of no use to a genealogist. However, when one of these internal codes is listed, along with your parent's name, under SSDI I would like to know what it means.   My dad didn't die in a Veterans Hospital but did work at the Veterans Administration years ago, in addition to being a civilian employee with the AF for many years and serving in the Army during WWII. I would like to know what VA stands for.</description>

      <title>Question about non citizen S.S. applications</title>

      <description>I have a question.&lt;br&gt;&lt;br&gt;My grandmother applied for Social Security in 1939.  I was told that she was not a citizen until 1952.  My grandfather became naturalized in 1939.  Was my grandmother able to apply for S.S. because my grandfather had been naturalized?  &lt;br&gt;&lt;br&gt;Just searching for background information.  Thanks to anyone who reads this or wishes to explain benefits.</description>

      <title>Not in SSDI internet search, but SS says she is listed</title>

      <description>My mother died in 1975.  I have searched the SSDI high and low looking for her death record to no avail.  Finally, I gave up and got a copy of her death certificate which lists her SSN.  A search of SSDI still did not turn up a listing.  So I went to the local SS office.  The clerk told me my mother was listed on their death index.  &lt;br&gt;&lt;br&gt;Question:  Why would she be listed on their index, but when I query on line I cannot find her?</description>

      <description>I've been trying to help my husband's half-siblings find their father, who "disappeared" in about 1952.  There are rumors of mental illness, but nothing definitive; it's as if he dropped off the face of the earth.  It occurs to me that he might have wound up as an unknown, somewhere.&lt;br&gt;&lt;br&gt;Does anyone know if SSA keeps numbers active indefinitely, unless they are notified of a person's death? I would think that there are a huge number of people they don't know are dead.</description>

      <title>No hits on name when listed on my SSDI CD#110</title>

      <description>After I enter name on "Search the SSDI" screen and also list the SS# 334-10-0388,  the next screen has thrown the SS#  10 in the Last know address field and the 334 in the SS Info Field.  I get no hits.  Even without SS# and just name I get no hits?  The name is on my purchased CD#110.&lt;br&gt;&lt;br&gt;I entered a known name because I was getting no hits when I entered other names I wanted information on.  I am a paid subscriber to Ancestry.com and this SSDI board.</description>

      <description>Hi all,&lt;br&gt;&lt;br&gt;I am writing from Jacksonville, Florida U.S.A. Looking for any information related to my mother born Sonia Vivienne Broomfield (deceased 1967 in England)or family. She was born in London on Jan. 18th 1933, daugther of Ernest Broomfield and Ivy. Her father Ernest had a sister named Elsie and a brother named George. Her father Ernest was the son of George Broomfield. A woman named Lilian Hitchcock married into the Broomfield family on July 18th, 1931 to George.&lt;br&gt;&lt;br&gt;My mother Sonia married an American soldier (last name Parson) in Italy then moved to the U.S. and gave birth to a daughter named Joy. Sonia then married a man named Jim Ferris whom she had two children (names and sex unknown). She then married Ronald Aubrey Hall and had a son named Scott Aubrey Hall born 2/17/60 and a daughter named Lisa Marie born 2/28/61 (me). She then married or lived with a man named Bobby Clouser and had a son named Bobby. I've found Sonia's children Joy and Bobby. She moved back to England (taking no children) in approximately 1966 where she eventually succumbed to a long illness with cancer in 1967. Sonia had sisters named Joy, Shirley Anne Fields (the well known English film actress) and a brother named Ernest (who was killed in a shooting accident).&lt;br&gt;&lt;br&gt;I am looking for my two half siblings from Sonia's marriage to Ferris, her U.S. Social Security number, where she was buried and any other information. Her nickname was Sunny. Any information is most appreciated.</description>

      <title>1800 Births in SSDI?</title>

      <description>Not being sure of the given or surname of an ancestor born in 1871, I decided to search ssdi by only entering  1871 in the "year of birth field". Was surprised to see a lot of names listed. So, I kept entering an earlier year and was amazed that some 33 people born in 1800 were listed with social security numbers!       <title>Numident is worthless</title>

      <description>I wouldn't bother trying to get a numident printout if I were you. I did it thinking it would have all the info that a regular SS form would- just 11 dollars cheaper. Wrong!!! There was nothing there that I can't find on an internet search. No father's name or Mother's maiden name. No place of birth either. It just shows the date of death, birthdate, and last residence. The same info you get for free here on Rootsweb in a SSDI search. So, now I will have to go and send in the $27 for the full SS search like I obviously should have done in the first place. I'll end up paying $43 for the info after all. So, don't make my mistake unless you have money to burn.&lt;br&gt;           If you do have money to burn, please email me and I will be willing to help you :)</description>

      <title>Place of death. Not on index. How do I find it?</title>

      <description>I am from the UK searching for information on Mable H Hansen. Believed to have been born in Leola, SD. Maiden name believed to have been Rinke. Born 21st Feb 1907. Died 23rd Aug 1989. Last known residence Clinton, Tn. SSN 480-18-xxxx (issued in Iowa). Death search result does not give place of death. Friends in Clinton have searched library for obituary, etc. but nothing found. I assume that she died in Clinton but maybe she didn't. How do I find out place of death? Can anyone tell me, please?
