They might have been raised in an orphanage or by non-family members who never passed the information to the couple and not known their parents' names. Records were not kept as acurately then. The couple may have not wanted to acknowledge who their fathers were. They may have just neglicted to fill out that part of the form. I have seen quite a few forms like that. Recordkeeping was not as precise 100+ years ago.
